Some patients seen by doctors and clinics affiliated with Mc­Kenzie-Willamette Medical Center may have had their personal data, including Social Security numbers, stolen by hackers.

Names, addresses, birthdates, telephone numbers and Social Security numbers for patients seen over the past five years at Heart Associates of Oregon, McKenzie Primary Care Associates and Summit Surgical Specialists were taken, the medical center said.

On Monday, McKenzie-­Willamette’s parent company, Tennessee-based Community Health Systems, said the cyber­attack involved records for more than 4 million patients nationwide.
