The governor is asking the health insurance exchange board to fire its chief information officer and chief operating officer.

If the board complies, that would make five leadership positions vacated after Cover Oregon has failed to deliver a website to allow Oregonians to apply and sign up for health insurance. And Gov. John Kitzhaber says he might not be finished with sweeping up Cover Oregon’s leadership.

Kitzhaber released a report from consultant First Data that detailed the massive failure of Cover Oregon’s website. In its 77-page page report, First Data described the rollout of the website as a textbook example of management failure, crippled by ineffective contracting procedures, lack of oversight, competition for control among state agencies and an ultimately misbegotten belief that lead contractor Oracle Corp. would be able to create a working website on time.
