A problem with the Oregon Department of Revenue’s phone system caused 644 faxed submissions to arrive incomplete.

“Taxpayers sending the faxes would have every reason to think they had a successful transmission, but we received a distorted or partial image,” Policy and Systems Manager Megan Denison said in a press release.

The agency can identify the senders on about one quarter of the submissions and will contact those people, Denison said. But anyone who faxed documents to either unit between Friday afternoon and Tuesday morning needs to resend their information.
